Master Student Record Form: Fire Instructor I

Have You Pre-registered?

All students preparing to take or retake any written exam MUST be pre-registered 10 working days prior to the scheduled test. Your registration may be completed by using this on-line form or by calling MFTE and requesting a form by mail.

On-line registration is simple and fast. Just provide the information below and indicate what date and location you want to take the exam. Then send the form to MFT&E by pressing the "submit" button, or print the form and mail it to:

Those failing to pre-register by the deadline will not be permitted to take the exam.

Students must bring their personal photo ID with them to the written test site. Any one of the following is acceptable:

1. Valid Maine driver's license
2. Maine State ID
3. Military ID
